Dear Parents/Guardians, Hello parents of __________________________, I am looking forward to an exciting year in helping your child become physically as well as health educated. It is my objective as a physical education teacher to provide all students with the knowledge, skills, attitudes, and behaviors needed to live healthy lives. I would like to get started off in the right direction and let you as parents/guardians become aware of the rules and injury report process of the classroom and gymnasium. All information on this form will be discussed with your child in class, however, I’m asking that you review the information with your child and sign the form in agreement. Your questions and concerns are welcomed through the information provided above.
Course Description The purpose of this class is to promote lifelong learning and to develop the total fitness of young men and women. Students will learn how to live healthy lives and to pursue lifelong fitness through physical activity. Focus will be on providing students with an appreciation of exercise and health. A major emphasis will be on the development of skills from within the individual, sports, and activities, which can become valuable recreational assets which helps promote total fitness in adult life.
Classroom Rules
1.) Be RESPECTFUL to all students and staff
2.) Be Productive, and Prepared
3.) Be responsible
4.) No food and drinks, except for water and it must be in it’s original container
5.) Participate and have safe fun!!!!
Gymnasium Rules
1.) Be appropriately dressed (attire is discussed below)
2.) Be responsible and respectful to all peers and equipment at all times.
3.) Gum, food and drink are not allowed in the gymnasium or classroom.
4.) Always wear rubber soled shoes when in the gymnasium.
5.) Participate and have safe fun!!!!
Materials Needed for: Classroom
1.) Notebook
2.) Pens/Pencils
3.) Ear buds (for certain assignments with the laptops)
Gymnasium
1.) Change of clothes. Gym clothes must still be within the school’s dress code policy (see handbook).
2.) Proper footwear. Shoes MUST have rubber soles. No sandals, boots, heels, flip flops etc… are allowed on the gym floor. Failure to wear proper footwear can result in the student losing points and not being able to participate. The most important thing is the safety of each student!
Injuries Report All injuries must be reported to the instructor immediately. The teacher will then handle and file the report with proper authorities. The teacher will notify the parent/guardian.
If there are any physical limitations that will hinder your child from participating in any activities, please inform the teacher with a physician’s note stating detailed information about the limitations. A physician’s note must include the following information:
Grading- “The Way to Get an A” 1.) Attendance- Students are required to attend and participate in every class. If the student misses a day of class, that student has to make up all assignments within 4 days.
2.) Dress Code: Students are required to change clothes when they are in gym. Wearing the clothes that you have worn to school for the day does not constitute changing out.
3.) Participation- Students are required to participate in every class. PE uniform and athletic shoes must be worn in order to participate and must fully participate to receive credit for the day.
4.) Skills Assessment/Tests/Written Assignments- Students will be assessed, tested, and/or given a written assignment after each taught unit. Students will only be tested on covered material.
Health Grades are equal to 50% and PE Grades are equal to 50% of the class.
The grading percentages are as follows:
Written Tests in Health- 20% Written Tests in PE- 20%
Assignments- 20% Fitness Journal- 20%
Daily Participation- 20% Dress-Out & Participation-20%
Cooperation & Effort- 20% Cooperation, Sportsmanship, Effort-20%
Projects/Papers- 20% Skills Test- 20%
